As publicised at our last AGM, SIN agreed to confirm its membership by asking all members to complete a simple form to comply with data protection issues.

Download this form as a PDF and print and complete it ensuring it is both signed and dated. Copies of the form should then be posted to John Coll at the address below or they can be delivered in person if you plan to attend the SIN event on Monday 27 June.

Please note that it is important that you complete the form! Failure to do will imply that you wish to be removed from our mailing list and you will not then be notified of any future events.
